I purchased the Perforance Products replacement carpet for my '87 
4Runner.  Overall I was pleased with the quality of the carpet.  It had 
no cutouts for shift or transfer case, and under the console.  It was 
quite a bit larger than the original so it did take some fitting.

Start at the back and work your way forward.  Have sharp scissors or a 
blade to cut the carpet, it's pretty thick and has good padding under the 
footwells.

I just replaced the floor mat in my truck with one from Newark Auto
Products. I found them on the web at http://www.newark-auto.com/
I was not interested in carpet, as I like to be able to leave my shoes
on while wheeling in the mud. And none of the replacement carpets I've 
seen looked like anything but a replacement. This is a rubber wall-to-wall
mat that comes in any color you want, as long as it is black. I am
thinking of getting the factory removable carpet mats (like Nick has)
to put on top of the mat while in town.

I emailed them and they told me that Pep Boys was their local retailer.
Not my usual parts store, but they ordered one for me at $80, and it came
within 2 weeks, even during the UPS strike. It did not have a pad 
underneath, so I bought a 4' x 6' roll of Hot Rod Insulation from a 
local hot rod shop, and cut to fit. The rubber mat fit and looks fine.
It even had the shift lever holes pre-marked in the right places.

By the way, that insulation is cool stuff. It has aluminum foil on one
side, with re-enforcement every 1/4 inch, like rip-stop nylon. The other
layer looks like recycled cotton rags. It is tough stuff, and can be
glued onto the metal surfaces, like firewall and inside of hood, to
reduce noise. I'm planning to get another roll ($23) and line the inside
of the door panels, under the hood, and above the headliner (if I can
figure out how to get it off.) Now that I can drive with the windows closed
(thanks to the A/C) I'm getting picky about the noise level.
